A couple of decades ago I spent some time on a volunteer EMT ambulance service. I've seen all sorts of fatalities when people are thrown from vehicles and when still in the car and not wearing seatbelts.
I've never seen a wreck as bad as this one with a belted survivor, but I'm not surprised. Being belted gives you your best chance.
I did see a truck where two drunks hit the concrete post that held a train crossing sign. Totaled the truck, didn't move the concrete more than an inch or two, killed the unbelted guy and the other got deep lacerations on his back.
