In the United States, the increase in life expectancy is heavily based on child mortality rates. 80-100 years ago, the trick wasn't so much living until 75 as it was surviving until one's 2nd birthday.

For those who cleared the first hurdle, the numbers weren't radically dissimilar from ours today.

That's very true. I recently finished a book called "The Drunkard's Walk" that included a study of mortality in old England. It was very clear that if you lived out of infancy, you could make it to (maybe) 40.
